Individual → Individual Sale

  • Valid picture ID
  • Completed LDR Form R-1331 (Boat Registration Tax Payment Certification)
  • Notarized bill of sale signed by both parties
  • Previous owner’s boat registration/title
  • Payment of taxes: Debit/credit, check, or money order

Dealer → Individual Sale

  • Valid picture ID
  • Completed LDR Form R-1331
  • Notarized bill of sale or dealer invoice
  • Payment of taxes: Debit/credit, check, or money order

Commercial Fisherman Exemption

  • Valid picture ID
  • Current federal income tax return + supporting schedules
  • Valid Louisiana vessel license from the Department of Wildlife & Fisheries
  • Proof of vessel ownership
  • Current commercial fishing license
  • Completed Form R-1334 (Commercial Fisherman Louisiana Sales Tax Exemption Application)
  • First-year applicants only: Notarized statement declaring no income from commercial fishing for the year

Requesting Copies of a Return or Other Documents

  • Valid picture ID
  • For businesses: Louisiana Department of Revenue account number
  • Completed LDR Form R-7004 (Tax Information Disclosure Authorization)
  • If you are a representative: Completed Power of Attorney Form R-7006
  • Payment for copies: Debit/credit, check, or money order

Gaming Clearance

  • Valid picture ID
  • Completed Individual Consent to Disclose Federal Tax Information / Clearance Form
    • This form is in your Louisiana Gaming Control packet (available from the Office of State Police, preferably filled out in blue ink)

IFTA (International Fuel Tax Agreement)

New IFTA Account

  • Valid picture ID
  • Federal Employer Identification Number (FEIN) if LLC
  • Completed LDR Form R-5678-L (Decal Request)
  • Payment:
    • Application fee: $35
    • Decal set: $2
    • Debit/credit, check, or money order

Existing IFTA Account – Decal Request

  • Valid picture ID
  • Louisiana Department of Revenue account number
  • Completed LDR Form R-5679 (Decal Request)
  • Payment: $2 per decal — Debit/credit, check, or money order

Video Poker, ATC, or Lottery Clearances

  • Valid picture ID

  • If not the business owner: Completed Form R-7006 (Power of Attorney)
